Housing Application
If you are an adult seeking supportive housing or structured living support in Northwest Indiana, complete this form to begin the eligibility review process. This form is for individuals applying on their own behalf.
Agency Referral Form
This form is for case managers, social workers, hospitals, shelters, VA programs, reentry programs, and other partner agencies referring an individual for housing support. Do not use this form if you are applying for yourself — use the Housing Application above.
Zero Income Certification
For applicants who currently have no income and need to certify their income status for eligibility review. This form is required in addition to the Housing Application when you have no current income. Complete both forms if this applies to you.
NWISLI primarily serves Northwest Indiana and surrounding communities. Applicants from outside this region may still apply, but placement priority is given to local applicants, and out-of-region placement is subject to housing availability and partner coordination. If you are located more than 60 miles from Gary, IN, please contact NWISLI before submitting to confirm program fit.
